Because both measurements are lengths, the units cancel — inches over inches gives the same ratio as centimetres over centimetres. There is no separate imperial formula.

Why waist-to-height rather than BMI?
BMI treats all weight identically. Two people with the same BMI can carry very different amounts of visceral fat — the deep abdominal fat that surrounds the liver, pancreas, and intestines and is metabolically active in ways subcutaneous fat is not.
Visceral fat releases inflammatory signalling molecules and free fatty acids directly into the portal circulation, which is the mechanism linking it to insulin resistance, type 2 diabetes, and cardiovascular disease. Waist circumference tracks it; total weight does not.
NICE now recommends using waist-to-height ratio alongside BMI for adults with a BMI under 35, precisely to catch the people whose BMI looks acceptable while their fat distribution does not.
Why divide by height at all?
Waist circumference on its own needs different thresholds for men and women, and different ones again for South Asian, Chinese, and Japanese populations, because a 35-inch waist means something different on a 5-foot frame than a 6-foot one.
Scaling to height largely removes that problem. The 0.5 boundary performs consistently across sexes and most ethnic groups without adjustment, which makes it both more accurate and far easier to remember than a table of cut-points.
How to measure your waist correctly
- 1.Measure on bare skin, standing upright with feet together.
- 2.Find the midpoint between the bottom of your lowest rib and the top of your hip bone — not your navel, and not where your trousers sit.
- 3.Wrap the tape horizontally, snug but not compressing the skin.
- 4.Breathe out normally and read the tape at the end of the exhale. Do not hold your breath or pull your stomach in.
- 5.Measure twice. If the readings differ by more than a centimetre, measure again.
Measuring at the navel — common in fitness contexts — reads differently and will not match clinical thresholds, particularly at higher body weights where the navel sits lower.

Crossing 0.5 is not a diagnosis. It is a signal to look at the things that actually drive risk — blood pressure, fasting glucose or HbA1c, and a lipid panel — which together give a far more complete picture than any measurement of shape.
Reducing central fat
You cannot target fat loss to the abdomen through abdominal exercise. Visceral fat does, however, respond well to a general calorie deficit — often faster than subcutaneous fat, so waist measurements typically improve early in weight loss.
Aerobic exercise reduces visceral fat even without weight loss, which is one of the clearer findings in the exercise literature. Sleep and alcohol both matter too: short sleep and heavy drinking are independently associated with central fat accumulation.
